It is very unlikely for the tcc clutch to fail and cause a lock-up on its
own. The clutch would have to be enabled and then jam internally in the
converter, I doubt it would ever disengage after that. An uncommon problem
that I've seen a couple times is the tcc valve in the pump sticking after an
apply, this keeps the converter locked up whenever the engine is running.
Those always get a new pump.
