Brian,

A bad MAF can cause those exact problems--but don't overlook a plugged EGR,
O2 biased rich, low fuel pressure (and volume) and wrong ignition timing or
plug heat range. There's been issue to the use of K&N filters (and their
over-oiling) causing MAF contamination. If metered air is incorrect--fuel is
also incorrect.
