commenting of disassembled code

Does anyone out there have a way to comment disassembled code, such that 
when the disassembler is rerun, the new output source will have all the 
previously entered comments?

If the disassembler itself doesn't do this, the comments could be put into 
a separate file manually, and have a separate little program put them into 
each fresh disassembly.  or there could be a program that strips the 
comments out of the previous disassembly, and plugs them into the new one.

Has anyone done something like this, or found a HC11 disassembler that has 
this feature?
