Measure the signal from the O2 sensor (not the wideband one but the one connected to the ECM) and make sure that the voltage really drops down to zero relative to the ECM's signal ground when mixture is lean (and goes up to at least 0,85 V at rich mixture). Possible causes could be bad ground connections for the ECM harness or bad grounding of the part of the exhaust where the O2 sensor is fitted.

most ecu's I have come across wont adjust if it thinks its on idle (switch on throttle)
or engine not warmed up fully...
