Car Dieing in Drive update/Any Ideas?

So far here is what has happened.

The torque converter has been reported to be ok, the clutchs were in
good shape.

Previous info:

Car dies at idle (when in D), car also appears to either be in the
wrong gear (sometimes) or the TCC is locked when it should not be.
The RPMS are too low sometimes when driving, and with a bit of
application of the loud petal will suddenly appear to "break loose"
and the the car will run like normally and the rpms go up to where
they should be.

The only thing that seems likely left is the tranny has a problem, and
that is going to be looked at. The tranny does have 123k, with about
200 1/4 runs, and maybe 5 hours of road racing.

Any other ideas on what would cause this sort of behavior?
